Niall's Pedal Board is a simple open source VST host intended for live use. As such, it allows you to assign joystick and MIDI events to plugin parameters, to let you manipulate them in real-time. My original intention with it was for it to be the software part of a pedal board project, where I took a cheap joystick apart, and turned it into a simple floor-based controller for my guitar, with everything running through my laptop. My aims have changed slightly since then, but there's an html tutorial included in the zip, showing how I made the floor controller.
